As Bali Dialysis Center; we act with the basic principle of protecting the privacy of private life, considering the fundamental rights and freedoms of our patients, their relatives, our employees and all persons in communication with us. Our institution safely preserves all kinds of your personal data, including your personal health data, by taking the highest level of administrative and technical measures within the scope of the T.R. Constitution and the Law on the Protection of Personal Data No. 6698 (KVKK).
This Illumination Text has been prepared by Bali Dialysis Center in the capacity of Data Controller; for the purpose of informing you transparently about which personal data of yours will be processed for what purpose, to whom it can be transferred, collection method and legal reason, and your rights under the Law.
For What Purpose are Your Personal Data Processed?
Your personal data (Identity Information, Contact Information, Location Data, Special Quality Personal Data such as Health Information, Biometric / Visual Data) are processed for the following purposes in order to carry out the specific dialysis treatment services we offer in an uninterrupted, qualified and correct manner in accordance with the legal Legislations:
- Full execution of medical diagnosis, treatment and care services in accordance with the Health Services Basic Law No. 3359, Ministry of Health legislations and Regulation on Dialysis Centers.
- Opening a patient file, completion of legal records to hospital information management systems.
- Performing necessary legal notifications to the Ministry of Health, SGK and other relevant public institutions such as E-Nabız, Medula, Merd-Sis (Dialysis Information System).
- Effective planning of special transportation and transfer services for our patients in accordance with their home or logistic addresses (Location and contact data).
- Management of financial processes with invoicing, SGK reconciliation and private health insurance companies.
- Ensuring facility security with camera recording systems and audit processes.
To Whom are Your Personal Data Transferred?
Bali Dialysis Center keeps your personal data confidential within the framework of full compliance with the legislation. When necessary for the provision of your medical treatment and the performance of legal obligations, it can be transferred to the following authorities, provided that the relevant legal rules are not violated:
- Republic of Turkey Ministry of Health, Provincial Health Directorates and relevant subordinate public institutions and organizations.
- Social Security Institution (SGK) and contracted private insurance/complementary insurance companies.
- Contracted external laboratory enterprises where laboratory tests are performed.
- Judicial authorities, courts and other legal authorities authorized by laws.
What are Your Rights within the Scope of KVKK?
In accordance with the 11th article of the Law, you can use these rights by applying to us in writing:
- Learning whether personal data is processed, requesting information if processed,
- Learning the purpose of processing the data and whether it is used in accordance with the purpose,
- Requesting correction of missing or incorrectly processed data,
- Requesting deletion or destruction of your data subsequently within the framework of the conditions stipulated in the law.
